The COVID-19 pandemic has generated many conflicting rumors about the impact of coronavirus infection on the course of cancer. The use of viruses in the treatment of malignant neoplasms is considered as one of the promising therapeutic strategies. To date, drugs based on the so-called oncolytic viruses have been developed and registered abroad.

However, clinical studies have shown a very modest effectiveness of viral preparations. The main problem is the targeted delivery of the virus to the tumor; therefore, local injections of viruses into tumor nodes are currently used, which limits the effectiveness of cancer treatment.

The COVID-19 pandemic has given rise to many conflicting opinions, usually based on isolated clinical cases, about the effect of coronavirus infection on both stimulation and inhibition of tumor growth. The use of infectious, including viral, agents for the treatment of cancer patients has a long history. Back in 1896, a case was published of the development of a temporary remission after an influenza infection in a patient with acute leukemia. However, these strategies have not demonstrated significant clinical outcome. It is very likely that in patients with COVID-19 against the background of a “cytokine storm”, which is one of the manifestations of systemic activation of immunity, some antitumor effects are possible. They can be associated not only with the activation of the antitumor link of immunity, but also arise due to the fact that the tumor node enters the affected area of ​​normal tissue, such as the lungs, and also undergoes destruction.

At the same time, we should not forget that the “storm” is replaced by “calm”, which is no less dangerous. In response to an infection, the body realizes all its defenses, releasing the accumulated reserves of biologically active substances, including cytokines – regulators of the immune function, after which it takes time to restore the lost. Against this background, immunosuppression is observed, up to the development of immunoparalysis, when the body is no longer able to restore the functioning of the immune system. This is confirmed by the fact that the severity of coronavirus infection is clearly combined with the degree of decrease in the blood of sick lymphocytes, the main participants in antitumor immunity. Against this background of inhibition of antitumor immunity, malignantly transformed cells have a chance to break through the weakened protective barriers and start growing. Therefore, coronavirus infection can hardly be considered from the point of view of antitumor therapy; rather, on the contrary, the systemic inflammatory response caused by the virus and the subsequent suppression of immunity can contribute to tumor progression.
